Responsibilities

  • Perform comprehensive in-home patient assessments and develop individualized care plans
  • Administer medications, treatments, wound care, and IV therapy as ordered
  • Monitor vital signs, disease progression, and response to interventions
  • Educate patients and families on self-management, medications, and safety
  • Coordinate care with physicians, therapists, and other providers while documenting all visits, assessments, and communications in the EMR

Compensation & Benefits
