📋 Daily Home Health Nursing Duties

  • Perform comprehensive in-home patient assessments and develop individualized care plans
  • Administer medications, treatments, wound care, and IV therapy as ordered
  • Monitor vital signs, disease progression, and response to interventions
  • Educate patients and families on self-management, medications, and safety
  • Coordinate care with physicians, therapists, and other providers
  • Document all visits, assessments, and communications in the EMR

Education

Active RN license (Associate or Bachelor of Science in Nursing)

Board Certification

Current BLS/CPR certification required

License

Valid New York RN license (or compact eligible with ability to obtain)

Experience

1+ years of recent Home Health or Med/Surg nursing experience

Reliable transportation, valid driver's license, and auto insurance
