WellBe Senior Medical is hiring a part-time CareConnect Registered Nurse for in-home medical care in the Toledo, Ohio market. The RN provides urgent and episodic nursing services during patient home visits, supports telehealth consults with providers as needed, and handles related documentation and care coordination.

Responsibilities

  • Assess patients during in-home encounters and identify changes that require provider follow-up.
  • Review medications, treatment plans, symptoms, and relevant patient history during visits.
  • Provide ordered nursing interventions, including medication administration, wound care, phlebotomy, point-of-care testing, catheter care, and IV fluid administration.
  • Evaluate home safety and document findings in the patient record after each encounter.
  • Coordinate follow-up care with APPs, physicians, nurses, social workers, and other members of the care team.
  • Educate patients and caregivers and support transitions between home, hospital, SNF, and community settings.

Licensure
Active RN license required; compact licensure preferred
Experience
3+ years of direct patient-facing RN experience
Certification
Current CPR certification
Driving Requirements
Valid driver’s license with acceptable driving record and active auto insurance
Work Readiness
Ability to conduct in-home patient visits and use telehealth and electronic documentation systems
Preferred Background
Home care, telehealth, hospice, palliative care, geriatrics, or complex patient care experience

Compensation & Benefits

Pay is hourly with mileage reimbursement for local travel. A sign-on bonus is included.
