Responsibilities

  • Welcome patients into continuous care programs and explain included benefits and services
  • Coordinate with the patient’s Respiratory Therapist and pulmonary providers to maintain a collaborative care approach
  • Educate patients on the frequency and proper use of assigned in-home monitoring devices when applicable
  • Create personalized comprehensive care plans with patients through phone or video visits and update them monthly based on readings
  • Review and evaluate in-home device readings in real time during normal business hours, document trends and symptoms, and escalate concerning findings to the provider per protocol
  • Serve as the patient’s primary contact for non-emergent needs such as medication refill coordination, appointment scheduling, and healthcare navigation support

Qualifications & Requirements
