Responsibilities

  • Perform routine and urgent bedside assessments, triaging consults by patient acuity and delivering patient and family education during visits.
  • Order, perform, and interpret laboratory and radiology tests within the scope of practice; prescribe medications including controlled substances as licensed and delegated.
  • Coordinate care for palliative referrals, consult with the palliative care physician or designees, and keep the primary physician informed of services provided.
  • Complete accurate clinical documentation within 24 hours of each visit and submit billing and coding according to CMS guidelines.
  • Participate in monthly interdisciplinary team meetings, communicate patient and family needs to the IDT, and assist in identifying interventions by other team members.
  • Supervise nurse trainees and other learners in field placements while supporting advance care planning discussions and goal-setting with patients and caregivers.

The role requires consistent collaboration with the assigned Medical Director and flexibility to meet agency needs, including after-hours and weekend on-call coverage when required.

Qualifications & Requirements

Candidates must hold active credentials that allow independent practice under a collaborative agreement in Georgia and demonstrate substantial clinical nursing experience relevant to complex and serious illness care.
