Responsibilities

  • Assess the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment, then communicate findings to the registered nurse and interdisciplinary group.
  • Conduct social evaluations in the patient’s identified residence and develop intervention plans based on those findings; assist when the current setting is unsafe and an alternative plan is needed.
  • Counsel patients and families/caregivers regarding stress and other identified coping difficulties connected to the terminal illness and end-of-life issues.
  • Provide information and referral services for practical and environmental needs, serve as liaison with community agencies, and maintain collaborative relationships with organization personnel.
  • Participate in the development of the individualized plan of care, attend regularly scheduled interdisciplinary group meetings, and assist the physician and team in understanding significant social and emotional factors related to health problems and death/dying.
  • Maintain clinical records on all patients referred to social work and actively participate in quality assessment performance improvement activities.

Additional duties may be assigned by the Clinical Supervisor as needed to support patient care and organizational standards.

Qualifications & Requirements

Candidates must meet the personnel qualifications outlined in federal hospice regulations and possess the education, experience, and practical credentials required for safe, independent practice in a home-based hospice setting.

Education
Bachelor’s degree in social work from a program accredited by the Council on Social Work Education
Regulatory Qualification
Meets personnel qualifications stated in §418.114(b)(3)
Experience
Minimum of one year of experience in health care; hospice experience preferred
Clinical Knowledge
Understanding of hospice philosophy and issues of death and dying
Certification & Skills
Current CPR certification; strong verbal and written communication and organizational skills
Transportation
Licensed driver with an automobile that is insured in accordance with state or organization requirements and in good working order
