Responsibilities

  • Manage a caseload of skilled nursing and post-acute residents from admission through discharge, monitoring clinical progress and updating plans as needs change.
  • Create and coordinate discharge plans that address resident goals, caregiver support, identified barriers, and required post-discharge services.
  • Collaborate with residents, families, nursing, rehabilitation, social services, physicians, payers, and external providers to support timely, safe transitions.
  • Coordinate follow-up services such as home health, additional rehabilitation, durable medical equipment, medications, transportation, appointments, and community resources.
  • Educate residents and families on discharge instructions, follow-up care, medications, treatments, and available supports.
  • Complete accurate case-management and discharge documentation and contribute to post-discharge follow-up, payer requirements, quality initiatives, and readmission-reduction efforts.

Daily work emphasizes interdisciplinary communication, problem-solving, and practical coordination that moves residents to the next appropriate level of care.

Qualifications & Requirements

Applicants must hold an active California LVN or LPN license and possess recent licensed nursing experience in skilled nursing, post-acute care, rehabilitation, discharge planning, or case management.

License & Education
Current, active California LVN or LPN license and graduation from an accredited nursing program
Required Experience
At least 2 years of licensed nursing experience in skilled nursing, post-acute care, rehabilitation, discharge planning, case management, or a related clinical setting
Coordination Experience
Background in interdisciplinary care planning, discharge planning, care transitions, or post-acute service coordination
Certifications & Skills
Current CPR/BLS certification; strong communication, organization, documentation, and problem-solving abilities; electronic health record proficiency
Preferred Qualifications
3+ years of skilled nursing or post-acute experience; familiarity with short-stay coordination, PDPM, Medicare/Medicare Advantage, payer authorizations, ACOs, PointClickCare, or readmission-reduction programs

Compensation & Benefits

Hourly compensation is $42–$52 per hour depending on experience.
