Licensed Practical/Vocational Nurse – Home Health in San Francisco, CA

Working as a Licensed Practical or Vocational Nurse in home health in San Francisco requires adapting skilled nursing practice to the private residence rather than a controlled facility environment. Without continuous on-site staffing or immediate access to hospital resources, the LVN/LPN must execute the RN-established plan of care reliably, observe for changes, and communicate findings promptly. Professional boundaries remain essential: interventions stay within the California LVN/LPN scope while patient and family education supports safe self-management between visits. Documentation becomes the primary record that links successive visits and informs the interdisciplinary team of therapists and aides. The mobile nature of the work across San Francisco neighborhoods further demands consistent infection-control habits, safe equipment use, and clear hand-offs so that care remains coherent as the patient recovers at home.
