Responsibilities

  • Conduct comprehensive in-home health evaluations that assess overall health status and well-being.
  • Identify barriers to care through structured clinical assessment in the member’s residence.
  • Review medication lists and medical history with the patient during the home visit.
  • Complete physical evaluations and enter findings into pre-populated, patient-specific digital forms.

Qualifications & Requirements

Candidates must hold current California credentials and meet the clinical preparation standards listed below. Prior Nurse Practitioner experience is preferred; strong new graduates may be considered.

Education
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
Board Certification
Board-certified Nurse Practitioner (ANCC or AANP)
Licensure
Active California RN and Nurse Practitioner licenses; Medicaid enrollment strongly preferred or willingness to obtain
Transportation
Valid driver’s license and reliable personal vehicle for travel within the service area
Additional
DEA registration is not required

Compensation & Benefits

Earnings are driven by completed assessments at an $80 base rate. Volume of 6–9 visits per day typically supports the stated weekly range, with additional multipliers and incentives available.
