Field Nurse Practitioner (FNP) – San Diego, CA

A Field Nurse Practitioner role in San Diego centers on mobile primary care delivered through programs such as Molina Care Connections. Rather than practicing inside a fixed clinic, the FNP travels to members in their homes and community locations to perform preventive assessments, identify gaps in care, and manage both acute and chronic conditions across adult, pediatric, women’s health, and geriatric populations. This model places particular emphasis on improving access for individuals enrolled in government-sponsored plans who encounter barriers to traditional office visits. Clinicians interested in community-based practice, independent decision-making within state scope-of-practice rules, and the coordination of follow-up services will find the work distinct from conventional outpatient settings. The position requires the full complement of Family Nurse Practitioner credentials and the ability to adapt clinical approaches to diverse living environments while remaining tightly integrated with a larger advanced-practice team.
