📋 In-Home Health Evaluation Responsibilities
As a contracted Nurse Practitioner, you will conduct comprehensive in-home health assessments for health plan members. These visits focus on understanding the whole person — reviewing medications, medical history, and performing a targeted physical evaluation — all within the comfort of the patient's home environment.
- Perform medication reconciliation and review of medical history
- Conduct physical assessments tailored to each patient's needs

Education
Master's or Doctoral degree in Nursing from an accredited program
Board Certification
