LVN Case Manager in Sacramento, CA

An LVN Case Manager working in community-based Enhanced Care Management operates outside the walls of a single clinic or hospital unit. In Sacramento County the role focuses on members whose medical conditions intersect with housing instability, behavioral health needs, or difficulty navigating multiple providers. The professional task is to keep clinical and social supports aligned so that care remains continuous rather than fragmented. This requires assessment skill, clear communication across disciplines, and practical knowledge of local resources. Unlike facility nursing that centers on shift-based procedures, the work emphasizes ongoing coordination and engagement with members in their own settings. For LVNs drawn to community practice and whole-person support, the position offers a distinct application of vocational nursing within California’s Medi-Cal care management framework.
