Geriatric Primary Care in an Outpatient Clinic Setting

Geriatric primary care Nurse Practitioner work in Sacramento centers on the clinical needs of older adults, many of whom present with concurrent conditions and changing functional status. The role emphasizes thorough clinical evaluation, individualized treatment of both acute and longer-term health concerns, and consistent monitoring that supports continuity of care. Medication management forms a core element of ongoing treatment, while education of patients and caregivers helps align daily care with clinical recommendations. Because the practice is clinic-based, encounters occur in a controlled outpatient environment that facilitates focused assessment and planned follow-up. Collaboration with other providers remains essential when a patient’s care involves multiple settings, yet the Nurse Practitioner’s primary clinical activity stays within the clinic. This combination of geriatric focus, primary care scope, and outpatient delivery defines the professional content of the position.
