The position is based at Squirrel Hill Health Center, a federally qualified health center with outpatient sites in the Pittsburgh area and a mobile medical unit. Responsibilities

  • Conduct patient assessments and physical exams, diagnose acute and chronic conditions, and develop individualized treatment plans.
  • Order and interpret diagnostic tests and imaging; analyze findings to guide clinical decisions.
  • Manage chronic disease states common in older adults, including diabetes, hypertension, dementia, polypharmacy, and falls risk.
  • Provide immunizations, patient education on prevention, diet, and hygiene, and perform minor procedures within privilege scope.
  • Coordinate care with specialists, behavioral health providers, social workers, and community resources; complete full documentation in the electronic health record within three working days.
  • Participate in quality improvement initiatives and HRSA/UDS reporting; take rotating after-hours call and complete assigned mobile unit or house calls as directed.

The clinician maintains an independent clinical practice that includes office visits while also covering mobile medical unit and home visit needs when directed by the Chief Medical Officer.

Qualifications & Requirements

Candidates must hold the credentials required for advanced practice in Pennsylvania and demonstrate readiness for independent primary care across the lifespan in a community health setting.

Compensation & Benefits

Compensation is provided as an annual range for this non-exempt advanced practice role.
