Responsibilities

  • Act as the main clinical point of contact for patients and families throughout the hospice episode.
  • Complete initial and ongoing assessments, both in person and by telephone, to track status and update the plan of care.
  • Provide hands-on skilled nursing care and symptom management according to each patient’s individualized goals.
  • Collaborate with physicians, nurse practitioners, LPNs, CNAs, social workers, and chaplains to refine care plans and manage level-of-care transitions.
  • Instruct caregivers on medication use, disease progression, nutrition, self-care techniques, and end-of-life expectations.
  • Record assessments, orders, progress notes, and care-plan updates promptly while reporting clinical changes to the appropriate provider.

Work also includes adherence to infection-control standards, regulatory compliance, and participation in quality initiatives or on-call coverage when required.
