Responsibilities

  • Provide skilled nursing services, treatments, diagnostic and preventive procedures according to the established plan of care and physician orders.
  • Observe patient signs and symptoms, document clinical and progress notes within 48 hours, and report changes in condition or reactions to treatments to the physician and Clinical Director or RN Case Manager.
  • Initiate and revise the plan of care as needed, prepare equipment and materials for treatments, and observe aseptic technique.
  • Counsel patients and family members on nursing needs and appropriate self-care techniques while referring to physical, occupational, speech, or social work specialists when specialized skills are required.
  • Submit daily schedule changes to the Director of Clinical Services or Nursing Case Manager and participate in patient care conferences, in-service programs, peer review, and utilization review as assigned.

All duties are performed under the direction and supervision of a Registered Nurse and in accordance with agency policies and procedures.
