Responsibilities

  • Deliver skilled nursing care to pediatric patients with complex needs, including those requiring tracheostomy, ventilator, or tube-feeding support.
  • Provide continuous one-on-one monitoring and clinical interventions during assigned shifts in the home setting.
  • Address holistic patient needs that extend beyond clinical tasks to include social, psychological, and cultural considerations.
  • Support families through education and coordination while maintaining the individualized care plan for each assigned child.
  • Document care accurately and collaborate with the clinical and operational support team as needed.

Nurses work independently with a single patient per shift, managing the full range of required interventions according to the child’s care plan and acuity level.
