Responsibilities

  • Perform comprehensive assessments including initial evaluations, annual wellness exams, follow-up visits, acute urgent visits, and post-discharge visits.
  • Educate patients and caregivers on disease management, preventive care, medications, and diagnostic testing.
  • Prescribe medications, order diagnostic tests, and perform point-of-care testing such as A1C and spirometry.
  • Coordinate care with primary care physicians, specialists, and ancillary services while collaborating with the Monogram physician.
  • Participate in recurring virtual integrated care team meetings and complete accurate, timely documentation.
  • Engage in quality assurance activities including audits and documentation reviews to maintain compliance and clinical standards.

The Advanced Practice Provider may also deliver care in affiliated settings such as skilled nursing facilities, assisted living facilities, or hospitals when applicable to patient needs.

Qualifications & Requirements

Compensation & Benefits
