Responsibilities

  • Obtain medical history, record vital signs, complete intake, and prioritize patient care according to clinical urgency.
  • Perform IV access, phlebotomy, vaccine administration, point-of-care rapid tests, IV medication and fluid administration, and EKGs.
  • Document patient information in the clinical EHR and in Salesforce for the membership team.
  • Maintain and track medication inventory; coordinate additional care with Aftercare, house-call, or medical-partner teams.
  • Support the clinical team within nursing scope of practice and participate in a house call when requested.
