Responsibilities

  • Evaluate hospitalized adults by obtaining histories, performing focused examinations, reviewing clinical data, and developing individualized assessment and treatment plans.
  • Order and interpret laboratory studies, imaging, and other diagnostic information; recognize changes in clinical status and escalate concerns appropriately.
  • Manage common acute and chronic medical problems through evidence-based pharmacologic and non-pharmacologic treatment within the applicable scope of practice.
  • Coordinate consultations, communicate with physicians and interdisciplinary staff, and contribute to diagnostic and therapeutic decision-making for medically complex patients.
  • Support admissions, daily inpatient management, transfers, discharge planning, medication reconciliation, and communication of follow-up needs across transitions of care.

The APP works as an integrated member of the hospital medicine service, balancing bedside clinical decisions with documentation, care coordination, patient communication, and collaboration with nursing, physicians, consultants, case management, and rehabilitation teams.
