Licensed to practice as an RN in Arizona
Nursing Experience
At least three years of nursing experience
Supervisory Experience
At least two years of supervisory experience in healthcare, social services, or primary care/family medicine
Clearances & Credentials
Valid Arizona driver’s license (or obtain within 30 days if out-of-state); Class I Fingerprint Clearance Card; current TB screening and immunization record; BLS certification within 90 days of hire
Preferred Knowledge Areas
Primary care nursing practice, case management, quality metrics (UDS, GPRA, HEDIS), and familiarity with FQHC and PCMH models
