Responsibilities

  • Perform thorough wound assessments and physical examinations during scheduled visits in patients’ homes.
  • Provide wound treatments according to evidence-based Wound Healing Guidelines and individualized clinical plans.
  • Complete accurate and timely assessments, clinical notes, wound measurements, and treatment documentation in the electronic patient record.
  • Teach patients and caregivers practical wound care, dressing, prevention, and healing-support techniques.

Qualifications & Requirements
