This full-time Primary Care Physician role is based within Christ Community Health Services' Memphis network of community health centers. Responsibilities

• Obtain comprehensive histories, perform physical examinations, and establish appropriate diagnoses and treatment plans
• Manage acute conditions, chronic diseases, preventive care needs, and routine primary care follow-up across the patient population
• Prescribe medications and provide patient counseling in accordance with Tennessee requirements and evidence-based clinical practice
• Perform appropriate minor office procedures, review diagnostic results, and coordinate referrals to specialists or other services
• Maintain timely clinical documentation and follow up on laboratory, imaging, EKG, and other diagnostic reports
• Participate in peer review, quality improvement, provider meetings, outreach activities, and organizational clinical initiatives

The physician works as part of an integrated care team and contributes to continuity, preventive care, chronic disease management, and quality improvement rather than functioning as an isolated office provider.

Qualifications & Requirements
