Responsibilities

  • Administer medications as ordered, including PRN doses, complete medication counts and reconciliation, and update the Medication Administration Record.
  • Obtain vital signs, collect clinical data within scope, and promptly report changes in physical condition, mental status, or safety concerns to the supervising RN or provider.
  • Provide direct supervision and support during assigned meal and snack periods in accordance with program guidelines.
  • Document all nursing observations, medication responses, incidents, and patient interactions in a timely and accurate manner.
  • Coordinate with the interdisciplinary team on admissions, status changes, and ongoing care needs.

All duties are performed under the supervision of a Registered Nurse or licensed provider and remain strictly within the LVN/LPN scope of practice.
