Responsibilities

  • Complete the comprehensive admission assessment and initiate the physician-ordered plan of care in the patient’s home.
  • Partner with the patient, representative, and caregivers to develop and revise the individualized care plan through ongoing interdisciplinary assessment.
  • Document clinical notes and report changes in condition to the responsible physician, obtaining new orders as needed.
  • Educate patients and family caregivers on the plan of care, preventive measures, and rehabilitative procedures.
  • Supervise and evaluate home health aide services every two weeks and provide direction to LPNs/LVNs and CNAs.

Additional duties include arranging necessary equipment and services, participating in quality assessment activities, and fulfilling on-call responsibilities defined by agency policy.

Qualifications & Requirements

An active Texas RN license and current CPR certification are required. One year of home health experience is preferred, and knowledge of Home Care Home Base is considered beneficial.
