Responsibilities

  • Develop and implement intermediate nursing plans of care and evaluate patient progress toward treatment goals.
  • Deliver direct patient care, administer medications and infusions, and educate patients and caregivers on prescribed therapy.
  • Assess physical status and needs at each skilled visit, re-evaluate therapy response, and secure physician order clarifications or revisions as needed.
  • Complete clinical documentation in real time during visits and prepare discharge summaries with patient instructions.
  • Mentor Infusion Nurse I staff when required and take part in multidisciplinary conferences and departmental on-call duties.

Additional duties include responding promptly to patient requests, initiating emergency procedures when necessary, and coordinating care with the clinical team, physicians, and families. When applicable, the nurse may insert PICC or Midline catheters, troubleshoot obstructed central lines, and educate on line care and infection prevention.

Qualifications & Requirements

Applicants must meet the following minimum credentials for safe practice in home and alternate-site settings.

RN License
Active and unrestricted registered nurse license in the state of practice
Nursing Experience
Minimum of one year of nursing experience
CPR
Current CPR certification
Transportation
Valid driver’s license, access to a reliable vehicle, and proof of liability insurance
Preferred
Bachelor of Science in Nursing and one to two years of prior infusion nursing experience
