Responsibilities

  • Perform skilled nursing care in the home, including medication administration, wound management, and ongoing patient assessments.
  • Create and carry out individualized care plans jointly with physicians and other clinicians.
  • Track patient condition, maintain accurate clinical records, and communicate changes to the physician and Clinical Manager.
  • Teach patients and families about disease management, medications, and treatment plans.
  • Supervise Home Health Aides and confirm that their documentation is complete and timely.
  • Coordinate overall patient care progress with the interdisciplinary team.
