Responsibilities

  • Perform comprehensive physical therapy evaluations to identify functional limitations and establish individualized treatment plans.
  • Deliver skilled interventions in accordance with physician orders and CMS Conditions of Participation.
  • Document evaluations, progress notes, and care updates accurately and within required timeframes.
  • Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention strategies, and home exercise programs.
  • Reassess patient progress regularly, adjust plans as needed, and communicate changes to the interdisciplinary team.

Clinicians work independently in the field while remaining closely coordinated with physicians, nurses, and other home health team members to support safe recovery at home.

Qualifications & Requirements

Candidates must hold active Michigan licensure and meet basic transportation requirements for home visits. Home health experience is preferred.

Degree from an accredited Physical Therapy or Physical Therapist Assistant program

Current Michigan PT or PTA license in good standing

Compensation & Benefits

Pay is competitive for the Detroit-area home health market and processed weekly. Benefits become available based on hours worked.
