Responsibilities

  • Provide direct clinical guidance to clinicians and respond to clinical calls from patients, caregivers, and providers, determining needed actions with clinical judgment.
  • Monitor daily on-call events, evaluate real-time assessment reports from clinicians, guide visit utilization, and follow up on verbal plans of care to ensure physician orders are timely and complete.
  • Complete daily transfer reviews for hospitalized patients, conduct root-cause analysis, and coordinate proactive interventions for high-risk patients.
  • Coach clinicians on clinical reasoning, visit planning, patient education, and documentation quality; facilitate new clinician orientation and lead skills validations before independent deployment.
  • Lead weekly case conferences, oversee Field Supervisors including matching and evaluating pairings, address performance issues with structured improvement plans, and serve as escalation point for clinical questions and high-risk situations.
  • Review Quality Assurance issues, communicate and educate clinicians on documentation accuracy, track HHCAHPS results and quality KPIs, maintain survey readiness, and perform field supervisory visits focused on infection control, safety, and clinical skills.

The Clinical Manager owns day-to-day clinical quality and is accountable for outcomes rather than activities alone.
