Responsibilities

  • Complete comprehensive patient assessments and required OASIS documentation for each assigned case.
  • Provide skilled nursing interventions with primary focus on wound management and infusion therapy.
  • Develop, implement, and revise individualized plans of care based on ongoing clinical findings.
  • Coordinate interdisciplinary services involving physical therapy, occupational therapy, speech therapy, medical social work, and home health aides.
  • Act as the central contact for physicians, patients, and families while delivering education on medications, treatments, and disease self-management.

Qualifications & Requirements

Compensation & Benefits

Compensation follows a productivity model with mileage reimbursement provided separately for all field travel.

  • Productivity pay of $64.10 per point with a target of 30 points per week, corresponding to annual earnings in the $95,000–$115,000 range
