📋 Daily Role & Clinical Responsibilities

• Perform comprehensive patient assessments including OASIS documentation
• Deliver skilled nursing care with emphasis on wound management and infusion therapy
• Develop, implement, and update individualized plans of care
• Coordinate care across disciplines including PT, OT, ST, MSW, and HHA
• Serve as primary point of contact for physicians, patients, and families
• Provide patient and caregiver education on medications, treatments, and disease management
