Responsibilities

  • Perform comprehensive patient assessments and complete OASIS documentation.
  • Provide skilled nursing interventions with particular attention to complex wound care and infusion therapy.
  • Create, implement, and revise individualized plans of care based on ongoing clinical findings.
  • Coordinate services across the interdisciplinary team, including physical therapy, occupational therapy, speech therapy, medical social work, and home health aides.
  • Serve as the primary clinical contact for patients, families, and physicians while educating caregivers on treatments and home management.

Compensation & Benefits

Pay follows a productivity-point model. Annual earnings scale with points completed, and mileage is reimbursed separately.
