Responsibilities

  • Perform comprehensive health-risk and needs assessments that examine psychosocial, physical, medical, behavioral, environmental, and financial domains.
  • Develop, document, and communicate individualized care plans used for service authorization, serving as the central contact to confirm services are rendered correctly during transitions and community-based support.
  • Implement and monitor strategies that address members’ social, physical, mental, emotional, spiritual, and supportive needs while identifying and closing gaps in care.
  • Review the plan of care at regular intervals, measure the effectiveness of interventions, and work with the interdisciplinary care-plan team to revise the plan when outcomes or circumstances change.
  • Educate providers, support staff, members, and families about the care-coordination role and member-focused health strategies; coordinate services across providers, members, families, and community resources to support continuity of care.
  • Produce reports that align with care-coordination goals, maintain professional contacts with inpatient, outpatient, and community resources, and assist with orientation of new team members as needed.
