Responsibilities

  • Receive transfers from the Patient Enrollment team and complete preliminary health assessments for newly enrolled patients.
  • Respond to patient questions and maintain open dialogue to identify individual needs and barriers to care.
  • Locate and arrange community resources that support the patient’s overall care plan.
  • Educate patients on chronic condition self-management and related health literacy topics.
  • Assist with medication management by identifying adherence issues, potential concerns, and coordinating refills.
  • Help ensure timely delivery of ordered services such as home health, durable medical equipment, and home infusion.

Compensation & Benefits
