Responsibilities

  • Conduct in-home Annual Wellness Visits and chronic condition assessments using standardized protocols.
  • Obtain focused histories, perform physical examinations, record vital signs, and complete medication reviews.
  • Carry out required screenings that include Hemoglobin A1C, spirometry, Kidney Health Evaluation (KED), Diabetic Retinal Exam (DRE), and Fecal Immunochemical Test (FIT).
  • Provide education to members on preventive care and chronic disease management.
  • Document each encounter in the EMR in support of value-based care and risk-adjustment requirements.
  • Practice within NP or PA scope of practice and applicable Ohio regulatory guidelines.

Training, equipment guidance, and EMR support are supplied so that visits remain consistent across providers.

Qualifications & Requirements

Applicants must meet state licensing standards and be prepared for independent field work.

Active, unencumbered Nurse Practitioner or Physician Assistant license in Ohio
State Practice Rules
Ability to practice under applicable Ohio guidelines, including supervision or collaboration requirements for PAs where required
Clinical Experience
Three or more years of patient-care experience preferred

Compensation & Benefits
