Responsibilities

  • Accept transfers from the Patient Enrollment team and complete preliminary health assessments for newly enrolled patients
  • Address patient questions and establish open dialogue to clarify individual care needs
  • Identify and coordinate community resources that support the patient’s overall care plan
  • Deliver patient education focused on chronic condition management and health literacy
  • Support medication management by reviewing adherence, noting potential concerns, and coordinating refills
  • Facilitate timely arrangement of services such as Home Health, durable medical equipment, and home infusion

The majority of the shift is spent on the telephone conducting assessments and coordinating next steps for patients entering the care management program.

Qualifications & Requirements

Candidates must hold an active multi-state compact LPN or LVN license and bring recent direct patient-facing nursing experience. Prior exposure to chronic care resource coordination is preferred.

Compensation & Benefits
