Responsibilities

  • Complete comprehensive patient assessments and OASIS documentation at the start of care and as required throughout the episode.
  • Provide skilled nursing interventions with primary emphasis on wound management and infusion therapy.
  • Develop, implement, and revise individualized plans of care in collaboration with the physician and interdisciplinary team.
  • Coordinate care transitions, communicate with physicians and families, and address barriers that could delay progress.
  • Supervise LVNs and home health aides as needed during the delivery of care.

Case management responsibilities center on maintaining clinical progress for high-acuity patients while ensuring accurate, timely documentation.

Qualifications & Requirements

Candidates must hold an active, unencumbered Texas RN license and demonstrate readiness for independent field practice with complex adult patients.

Compensation & Benefits

Compensation is productivity-based at $64.10 per point with an expected target of 30 points per week, producing an annual range of $85,000–$105,000 at target. Additional points completed generate additional pay.

Schedule & Work Environment

Weekly Schedule Full-time Monday–Friday with occasional weekend coverage as needed
