This position is based in Austin and covers patient homes throughout the greater Austin area and surrounding Central Texas communities. Care is delivered exclusively in the home setting through a Medicare-certified home health agency that functions as a service line of St. David’s HealthCare, itself part of HCA Healthcare.

Responsibilities

• Complete comprehensive physical therapy evaluations that assess mobility, strength, balance, gait, and functional limitations within the patient’s actual home environment.
• Design and update individualized treatment plans aimed at restoring independence, improving safety, and addressing pain or movement restrictions.
• Provide hands-on interventions such as therapeutic exercise, gait training, balance retraining, and functional mobility practice during each visit.
• Document each encounter promptly in the electronic medical record, including OASIS-related elements when required, and adjust the plan based on observed progress.
• Instruct patients and caregivers on home exercise programs, adaptive techniques, and environmental modifications that support continued recovery between visits.
• Coordinate care with the referring physician, nursing staff, occupational therapists, and other team members to maintain consistent goals and transitions.

Daily work centers on direct patient treatment in private residences rather than clinic or hospital therapy gyms. Clinical decisions are made at the point of care with real-time observation of how the patient functions in their own living space.

Qualifications & Requirements

Candidates must hold active credentials that allow independent practice in Texas home health and possess the practical ability to travel to multiple locations each day.

Compensation & Benefits
