Responsibilities

  • Conduct medical rounds on assigned Kaiser Permanente members at least twice weekly, addressing active clinical issues and supporting forward care planning.
  • Direct the on-site interdisciplinary group that includes a nurse practitioner and KP Acute Transitions Case Manager to verify appropriate level of care and steady progress toward defined goals.
