Responsibilities

  • Develop, implement, revise, and evaluate individualized patient-centered care plans under physician orders
  • Provide skilled nursing interventions and case management for an assigned group of patients
  • Perform initial evaluation visits and ongoing reassessments of nursing needs according to state and payer requirements
  • Document clinical notes, progress notes, OASIS and other required data sets within required timeframes
  • Educate patients and families on home care techniques, medications, and community resources; perform medication reconciliation
  • Coordinate with physicians, therapists, ancillary staff, and external agencies; participate in case conferences and report changes in condition

The RN also supervises and teaches other nursing personnel as assigned, processes orders, refers to therapy disciplines when indicated, and may take on-call duty nights, weekends, and holidays.

Qualifications & Requirements
