Responsibilities

  • Deliver medical and personal care according to the individualized Plan of Care prepared by the Nurse Supervisor and kept in the client’s home.
  • Orient new caregivers to assigned cases and the client’s Plan of Care so clinical needs continue without interruption.
  • Report changes in client medical status or emergencies promptly to the Nurse Supervisor.
  • Notify Staffing Coordinators of any schedule changes, including tardiness, time-off requests, or reductions in hours.
  • Drive or escort clients to appointments and activities when requested, using the client’s vehicle and ensuring safe, timely transport.
  • Confirm weekly schedules with the Staffing Coordinator and complete accurate, timely documentation required for case management and regulatory compliance.

Additional duties may be assigned as case needs evolve.

Compensation & Benefits
